Personally appeared John Hartung
of Dickeyville Wisconsin whom I certify to be respectable and entitled to credit, and who being by me first fuly sworn, according to law, does depose and say that he is _________ years of age;
"And that he is the claimant in Pension Claim No 596842 John Hartung Private in Co. F. 45 Reg. Ill Inf. Affiant says he was treated at hospital or Soldier's Home at Chatanooga Tenn from about Jan. 1st to about the month of February or first of March, 1865 when he was sent to join his Reg. via Baltimore, MD. and did join the Reg at Goldsboro NC. on or about the 24 of March 1865.
"Affiant says he got Medicene from the Regimental Surgeon quite often for said disease but he does not remember the name of the surgeon nor has ever seen him since his return from the Army nor does he know his present address nor his present place of residence.
"Affiant also says he has not seen any of the commissioned officers nor any of the Sergeants of his Co. since his return from the Army nor does he know their present address.
"Affiant also says that at the time he contracted said disease to [wit; ?] on or about Dec. 25, 1864 the Battalion to which he belonged at its time was composed of recruits for its 45 Reg. which had not as yet joined its said Reg. but had been attached to its Army for the Battle of Nashville and then followed its Rebel Army to said place."
and that his Post Office address is Dickeyville County of Grant State of Wis. and he further says that his knowledge of the above facts is obtained from the following source, viz.: __________being its Applicant_________ and that he has no interest in the prosecution of this claim.

Sworn to and subscribed before me this 18th day of May A.D. 1887 and that I hereby certify that I have no interest nor concern in the prosecution of said claim, and that I read the foregoing to deponent, and that he has subscribed and sworn to the same with a full knowledge of its contents.
