Declaration for an Original Invalid Pension
This must be Executed before a Court of Record or some Office thereof having Custody of the Seal.

State of Illinois, County of Jo Daviess, SS:
On this 9th day of February A.D. one thousand eight hundred and eighty7 personally appeared before me a clerk of the Circuit a Court of Record within and for the county and State aforesaid John Hartung aged 59 years, who, being duly sworn according to law, declares that he is the identical John Hartung who was ENROLLED on the 13 day of October, 1864, in Company F of the 45th regiment of Ill Inf. commanded by Lieut. Joseph R. Baxter and was honorably DISCHARGED at Louisville, Ky on the 12 day of July, 1865, That his personal description is as follows: Age 37 years; height 5feet 7inches; complexion dark; hair Brown; eyes Hazel. That while a memeber of the organization aforesaid, in the service and line of duty at Decaturs [sic] in the State of Alabama on or about the 27 day of December, 1864, he "Chontracted [sic] Chronic Bronchitus and Asthma caused by a severe cold contracted from fording a large stream of water which was about five feet deep, near Decaturs [sic] Ala which was either the Tennesee [sic] River or a Branch of the Tennesee [sic] River close to Decaturs Ala. Affiant is not sure which."

That he was treated in hospitals as follows: In Soldier's Home at Chatanooga [sic] Tenn. in which place he was treated for disease.
That he has not been employed in the military or naval service otherwise than as stated above ____________________________________________ That he has not been in the military or naval service of the United States since the 12 day of July 1865. That since leaving the service this applicant has resided in Dickeyville of Grant Co in the state of Wisconsin, and that his occupation has been that of a Farmer Shoemaker That he is now wholly disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints with full power of substitution and revocation.
Moses Rees of Galena Illinois his true and lawful attorney to prosecute his claim. That he has not received nor applied for a pension; that his residence is Dickeyville, Grant Co. Wisconsin and that his port office address is Dickeyville Grant Co. Wisconsin
